Claim Information

To receive a payment, you must complete the information below and provide the requested documentation.

  • To establish your eligibility to participate in this Settlement, please provide proof that you sent or deposited fiat currency into an FTX- or Alameda-related account at Silvergate Bank.

  • To verify your FTX Bankruptcy Claim amount, please provide a copy of the communication you received from the FTX Bankruptcy showing your name and Claim Amount.

  • If you are filing on behalf of an entity or otherwise on behalf of the Class Member, please include documentation showing that you are authorized to file on their behalf.

Supporting Documentation

Using the uploader below, you must upload documentation, establishing your eligibility, confirming your FTX Bankruptcy Claim Amount, and if applicable, your evidence of authority to act on behalf of the Claimant. “Self-prepared” documents such as handwritten receipts are, by themselves, insufficient to receive reimbursement, but can be considered to add clarity to or support other submitted documentation.

Providing documentation for FTX Bankruptcy Claim Amount does not guarantee that you will be entitled to receive the full amount claimed. All Valid Claims will also be subject to an aggregate maximum payment amount. If you would like to learn more, please review the Settlement Agreement.

Please provide only copies of your supporting documents and keep all originals for your personal files. The Settlement Administrator will have no obligation to return any supporting documentation to you.

  • The document uploader will only accept the following file types: .pdf, .png, and .jpeg.

Payment Election

Please select one of the following payment options, which will be used should you be eligible to receive a settlement payment. Please remember that the Court in charge of this case still has to decide whether to approve the Settlement. Payments will be made once the Court approves the Settlement and after appeals are resolved. Please be patient.

IMPORTANT NOTES:

  • For PayPal, Venmo, and Zelle payments, if there are any discrepancies, if the account is no longer active, or if the Settlement Administrator deems that payment cannot or should not be made based on the provided information, the Claims Administrator will either contact you for clarification or be authorized to default to a Check payment.

  • If domestic or international payment limits apply to your payment, you may receive more than one transmission or you may default to a Check payment.

  • Venmo and Zelle payments cannot be made to accounts outside of the United States.

  • Payments by Check will be made out to the Name on this Claim Form and mailed to the address on this Claim Form.

By signing below and submitting this Claim Form, I affirm that the information I provided in this Claim Form and my supporting documentation are true and correct to the best of my knowledge. I also affirm that I/the Claimant did not receive a postcard, and I have submitted proof that I/the Claimant sent or deposited fiat currency into an FTX- or Alameda-related account at Silvergate Bank.
